    When?    September 7-8, 2019

    Where?   RMU Island Sports Center

    Price?    $1,400 per team; non-refundable deposit of $300 is due immediately to confirm registration;
    ​               remainder is due no later than Sept 1st 

    How?    Complete the form below and pay by:
                  (1) Use the PayPal link on our Donations tab and add a comment "2019 Heitz of Heaven Hockey Tourney"
                  (2) Send a check to Heitz 61 Memorial Foundation, P.O. Box 54045, Pittsburgh, PA  15244
                      Teams are confirmed by order of full registration.  
                  Full registration includes submitting the information below and your payment.
                  Registration deadline is July 31, 2019
