2017 Heitz of Heaven Hockey Tournament - Team Registration Form

When? September 8-10, 2017

Where? RMU Island Sports Center

Price? $1,400 per team; non-refundable deposit of $300 is due immediately to confirm registration;
  remainder is due no later than Sept 1st

(2) Send a check to Heitz 61 Memorial Foundation, P.O. Box 54045, Pittsburgh, PA 15244
   Teams are confirmed by order of full registration.
   Full registration includes submitting the information below and your payment.
   Registration deadline is July 31, 2017
